About Future Alliance Day Support

Our services include a center-based, community-based, and a combination of both activity programs. Experienced staff with a Licensed Practical Nurse (LPN) on the premises. These distinctive multi-level programs have been structured flexibly to fit the needs of individuals. While our programs incorporate skill building components differently, each is geared towards supporting individuals to achieve personalized goals.

Eligibility

At Future Alliance, we serve individuals with (MR) diagnoses whom are 18 years of age or older. Those whom meet the eligibility criteria must have MR/ ID  or day support waivers. Please contact us if you have questions about a potential client’s eligibility or view this informative guide.
